Eligibility:
- Athletic equipment is a separate type of request from athletic registration.
- Athletic Equipment/Uniform requests will be considered for sport/team/dance troupe-specific equipment/uniform/costumes only, i.e., helmets, pads, cleats, team uniforms, karate gi/judo gi, dance shoes, recital outfit, etc.
- If AFFCF is not funding the activity for the child, supporting documentation is required to show that the child is actively participating in the sport.
- Items such as gym shoes, socks and general sports clothing may be requested under the category Supplemental Clothing. Requests for basketball hoops, soccer goals, volleyball nets, etc. cannot be considered.
- A request for athletic equipment/uniform needs to be for a business/organization whose information, services and enrollment/use of services are accessible to the public.
- Frequency is one time per year per sport.
Documentation:
No documentation required beforehand if AFFCF is paying for the sports registration. If AFFCF is not paying for the sports registration, documentation is needed showing the youth is currently enrolled in the sport.
Payment:
Once a request is approved, AFFCF sends an e-mail to notify the applicant that the request has been approved and for what amount so the purchase(s) can be made and the receipt(s) mailed to AFFCF for payment. The maximum AFFCF can consider for athletic equipment/uniforms is $200 ($100 for swimming lesson gear), which includes $100 maximum for shoes.
