Eligibility:
AFFCF can consider funding an instrument rental for one, two or up to three months at a time. Once the initial request is approved, renewals can also be submitted for a maximum of up to three months at a time – as long as the child remains a current ward of the court in Arizona and wishes to continue renting the instrument.
Documentation:
Documentation is required beforehand. Supporting documentation must show the vendor, the instrument and the cost per month.
Payment:
Once a request is approved, a check payable to company can be mailed or an e-mail will be sent so then the purchase can be made and receipt(s) submitted for payment. Frequency: 3 month at a time ongoing
